As soon as the ball hit the bat, which he WAS swinging at, it was fair and he became a "runner". It never went foul because it hit his noggin..... The following rule then applies:
"If a runner is hit by a fair batted ball while he is on fair territory, he is out and no judgment of intent is required unless he is hit by a deflected ball, or the ball has already passed all infielders, in which case the umpire must decide if he intended to be hit to interfere, obstruct, impede, hinder or confuse the defense."

He was in the batters' box. It's a foul ball, same thing if it were to hit his foot or any other part of his body. In fact it isn't foul, it's a dead ball and foul.

If you look further into the rules you'll find the so-called "batters box" is not considered in this, but homeplate, like any of the bases, is consider fair play ground and the ball is fair on it or forward of it and fair when it comes off the bat.

The idea IS to hit the ball off the bat. It can go fair or foul. It didn't go foul, it hit him in the face so he's out. The ball actually ended up in fair ground but he was already out. If it missed his face it would probably be foul.

Actually he's not out. He is still in the batter's box. It's no different than fouling one off your foot or ankle which happens more frequently.

If it hit's the bat, whether or not the batter swung, and then off any part of the body and goes forward the batter is out. This is little league and that is in the little league rules. Don't know about the NBA.
