The exit wound was on the top of his head, and it was most likely a pretty clean, small hole. But the shear wave would have turned the inside of his head to mush. A huge amount of the body's blood is devoted to the brain (just think of the size of the carotid artery and jugular vein). You'll get a big gush of blood from that.

As said in a post of above, the gun appeared to be a .22 caliber handgun. .22s used to be assassins handgun of choice for execution style killings because the bullet has enough power to enter the skull but enough not to penetrate the bone where the exit wound would be with a larger caliber gun. Thus the bullet ricochets and fragments around inside the skull until it comes to a stop. I'd say that's what happened in this video (which is an awesome visual if you have a good imagination ).
